Download, print, and complete the Camper Health History form
Download, print, and complete the Camper Healthcare Recommendations for Licensed Medical Personnel form (this form requires a Doctor's signature!)
Once the health forms are completed, you can upload them and complete the remaining required forms via this online Jotform.

We encourage you to submit the camper forms as early as possible, but forms must be received two weeks before your camper’s session check-in date. Campers who are attending more than one session only need to submit the forms once.

Use this form for Troop participation in a Service Unit Camporee/Ingathering at Camp Nocate or Camp Welaka. Completed forms must be returned to the Service Unit Camporee Chair or her/his designee prior to the event.

This form is designed for completion by the official Service Unit Camporee Chairperson to capture all girl, certified volunteer, and adult chaperone participants for one specific Camporee, as well as key planned activities.
